SELF PAY
You will be contacted prior to your surgery with an estimated procedure cost for your surgery. Payment in full must be received 10 days prior to surgery. You will be asked to complete a financial agreement. If the surgery performed is different than the surgery your physician has proposed, or there is an additional procedure performed, you will receive a statement for the balance due. If a refund is due, you will receive a refund check. If you receive prosthetic implants during your procedure you will be balance billed the cost of the implants used..
If your insurance company is not listed, it may be considered to be part of one of the networks listed above. Please call our office at 860-247-5555 and ask to talk with the billing department for more information.
